The application form for Ukraine must be completed online. Please ensure all questions are answered in full, and all names and details exactly match those in your passport. When prompted to select the consular office which will be processing your application, please click “Embassy of Ukraine in the UK” (there are two consular offices; this is the correct one for us to lodge your application in London). Once you have completed the online form, print it out, then sign and submit to us along with the other application requirements. Please do not process any payment; we will handle this on your behalf.
Please note, the embassy requires that you upload a scanned copy of your passport photograph along while completing the application form. If this is likely to be a problem, we can fill out the form and scan documents on your behalf, for an additional fee. Please see our Services page for further information.

Non-UK passport holders must submit an original UK residence card/permit (valid for more than 180 days per calendar year), or have proof of residence in their passport.

It must include the applicant’s full name, date and place of birth, nationality, passport details and place of residence, plus the purpose and duration of the intended visit, number of entries and place of stay in Ukraine. It should also confirm source of financial responsibility for your stay in Ukraine.
