Please print the application form on a single sheet of paper (front and back).
:: Proof of residence
Non-UK passport holders must submit an original, valid UK residence card/permit, or have proof of residence in their passport.
:: Employer’s letter
Please submit a letter from your UK employer, or school/university if you are a student. This must be an original, signed document on company letterhead, confirming your employment (or academic) status. It must note the purpose and dates of travel, and indicate the date on which you are due to return to work or study.
Self-employed applicants should instead submit evidence of their business activities and financial standing, e.g. HMRC tax return(s), company registration and/or a letter from your accountant.
Applicants who are unemployed or dependent must provide evidence of a family member or friend who can act as a sponsor, e.g. proof of ID, evidence of relationship to you (e.g. marriage or birth certificate), plus proof of immigration status if they are not a UK national.
:: Proof of funds
Applicant must provide bank statements covering the last three months, with the most recent showing a minimum credit balance of £600, OR traveller’s cheques to the equivalent value.
:: Proof of travel
Please submit your flight itinerary, showing arrival and departure dates to/from Malaysia.
:: Proof of accommodation
This should either be a hotel booking confirmation OR a letter of invitation if you are staying with friends/family members.
Where relevant, the letter of invitation, it must give full details of your contact address and telephone number, plus details of your relationship with the inviting party. It should confirm the purpose of your trip and provide details of where you will be staying while in Malaysia. If the inviting party is a Malaysian citizen, they must provide a copy of their IC (front and back); if on a student pass, they must provide a copy of their passport, including valid permit, and details of the college/university they are attending; if on an employment, diplomatic or official pass, they must provide a copy of their passport and work ID plus payslips, bank statements and employer’s letter confirming their position within the company.
